Responsibilities:
- Conduct assessments to identify clients’ needs, challenges, and strengths.
- Develop and implement individualized service plans based on client assessments.
- Provide counseling, guidance, and emotional support to clients and their families.
- Advocate on behalf of clients to access social services, housing, healthcare, or other needed resources.
- Collaborate with other professionals, agencies, or community organizations to coordinate services for clients.
- Maintain accurate client records, progress notes, and documentation in compliance with regulations.
- Monitor and evaluate clients’ progress, adjusting service plans as needed.

Qualifications and Skills:
