Responsibilities:

  • Conduct assessments to identify clients’ needs, challenges, and strengths.
  • Develop and implement individualized service plans based on client assessments.
  • Provide counseling, guidance, and emotional support to clients and their families.
  • Advocate on behalf of clients to access social services, housing, healthcare, or other needed resources.
  • Collaborate with other professionals, agencies, or community organizations to coordinate services for clients.
  • Maintain accurate client records, progress notes, and documentation in compliance with regulations.
  • Monitor and evaluate clients’ progress, adjusting service plans as needed.
  • Participate in community outreach, education, and awareness programs.

Qualifications and Skills:
