Responsibilities:
- Conduct comprehensive assessments of families’ needs and challenges through interviews and home visits.
- Develop individualized service plans for families, setting achievable goals and action steps.
- Provide counseling, guidance, and emotional support to families dealing with a range of challenges, including but not limited to financial difficulties, parenting issues, substance abuse, and domestic violence.
- Collaborate with community resources, social service agencies, and other professionals to connect families with appropriate support services such as housing assistance, healthcare, job training, and mental health resources.
- Offer parenting education, workshops, and skill-building sessions to enhance family dynamics and child-rearing techniques.
- Monitor families’ progress towards their goals, making adjustments to service plans as needed.
- Maintain accurate and up-to-date case records, documenting all interactions and interventions in accordance with agency standards and confidentiality requirements.
- Identify potential risks or safety concerns within families and take appropriate actions, such as reporting to authorities in cases of child abuse or neglect.
- Participate in team meetings, case conferences, and training sessions to stay updated on best practices and agency policies.
- Advocate for families within the community and social service systems to ensure they receive the necessary support and resources.
- Provide crisis intervention and support during emergency situations, offering immediate assistance and referrals when necessary.
- Uphold the agency’s mission and ethical standards while promoting the well-being and empowerment of families.
Qualifications:
- Bachelor’s degree in Social Work, Psychology, Sociology, or a related field. Master’s degree is a plus.
